Presenter(s): Gary Bess, M.S.W., Ph.D., Brenda Freeman, Ph.D., LCPC, CPC, NCC, and Jim Myers, M.S.W.
In this webinar, attendees will learn about the components necessary for developing a sustainability plan for integrated care and the process by which a plan can be created. The presenters will discuss the importance of periodic review based on self-reported metrics.
Objectives:
1. Examine the factors relevant to program sustainability.
2. Compare and Contrast various sustainability domains.
3. Develop a sustainability plan
Credits: 1 hour of ACCME, ANCC, AAPA, ASWB, APA
To receive a certificate of continuing education, you must electronically sign-in for the session, participate in the entire live internet activity and complete the online evaluation. You will be able to print your certificate after completing the evaluation. The evaluation for this session will close two weeks from the date of the webinar.

Dr. Bess founded GBA in 1991, and since that time the firm has provided consultation services for public and private health and human services organizations. The services provided include program evaluation, needs assessment, grant writing, economic analysis, strategic planning, GIS Mapping, and applied research. GBA provides services to several American Indian and Alaska Native programs, agencies and communities. Dr. Bess holds masters’ degrees in social work and applied sociology, and a doctorate in social work from the University of Southern California. Brenda Freeman, Ph.D., LCPC, CPC, NCC (Professor of Counseling and Educational Psychology and an Extension Mental Health Specialist @University of Nevada) Brenda Freeman lives in Reno, Nevada, and is a Professor of Counseling and Educational Psychology and an Extension Mental Health Specialist at the University of Nevada, Reno. She is honored to have had the opportunity to provide program evaluation and technical assistance to American Indian/Alaska Native communities for more than 20 years, including working with integrated care for the past 5 years. Brenda earned her Doctorate in Counselor Education and Supervision from the University of Wyoming and is a licensed mental health provider in two states. She has authored more than 50 journal articles and monographs, several of which are related to American Indian/Alaska Native culture, mental health, and disparities in access to care and educational opportunities. Jim Myers, M.S.W. (Macro Practice Social Worker @School of Social Work at California State University) Jim Myers expertise is in program evaluation and applied research activities. As a macro practice social worker, he is experienced in survey development, data coordination, data organization and analysis, and development of interim and final reports. Mr. Myers teaches research and program evaluation in the School of Social Work at California State University, Chico. Jim has more than 15 years of experience providing evaluation services to American Indian and Alaska Native programs/agencies/clinics and communities. He has master’s degrees in social work from California State University, Chico and has taken doctoral courses in human and organization development.
